Have a copy of this form printed for each SETT meeting and SETT follow up meeting(s)

Have a couple of copies of this form available for the SETT meeting and decide who will scribe on this document ahead of time. Can be completed either electronically or on paper. Share a copy of the completed form with all of the people that are at the SETT meeting as soon as possible after the meeting.

This document is helpful to provide the team will additional information beyond forms A and B. It is helpful-but not currently required.
